Disfunción renal: fármacos, medios de contraste radiológicos y sistema renina angiotensina

Resumen


El continuo envejecimiento poblacional, unido al incremento de la prevalencia de enfermedades crónicas no trasmisibles, entre las que se destacan la diabetes mellitus y la hipertensión arterial, como principales causas de deterioro de la función renal en las personas, ha hecho que con el paso de los años se introduzcan nuevos fármacos, más potentes, para la prevención y tratamiento de dichas entidades, lo que obliga a estar al día en el conocimiento de las características farmacocinéticas de estos medicamentos para evitar posibles efectos tóxicos, esto implica hacer una valoración previa de la función renal antes de introducir cualquier sustancia que pudiera ser potencialmente nefrotóxica. La máxima expresión de esta nefrotoxicidad lo constituye el fracaso renal agudo hospitalario o en la comunidad; en ambos casos, provocados por un error terapéutico, debido al mal empleo de drogas en pacientes ancianos con múltiples factores de riesgo o con un deterioro previo de la función renal. Es el objetivo de esta revisión bibliográfica ofrecer a los profesionales de la salud una visión resumida y actualizada de los mecanismos fisiopatológicos implicados en la nefrotoxicidad y a su vez, brindar las herramientas necesarias para el ajuste de dosis de los fármacos según la función renal del paciente.
